Patient Policies
Welcome to our practice and thank you for choosing us to serve your health care needs.

This website is designed to answer many of the questions that you may have. The more you know about our policies and methods of practice, the better we may be of service to you. Should you have any questions, please do not hesitate to contact us. It is our goal to provide you and your family the best medical care possible and to maintain a pleasant and long-lasting relationship with each of you.

Appointments

Appointments can be made by calling our practice directly. Alternatively, you can fill out the Request for an Appointment form on this website. Please bring your current insurance card, referral form if required, a list of any prescription you are currently taking or refills needed and a list of any questions you may have. If you would like to ask us any questions before your appointment, please fill out the Request Info form to help assist you during your visit.

Physician Orders
Please be sure all lab orders, biopsies, ultrasounds, and MRI’s are done 10 days prior to your follow-up appointment to ensure that we will receive the results timely. We will discuss your results during your follow-up visit. If you require to have lab orders done prior to your next visit, please be sure you keep your lab slip in a secure location. If you misplace your lab order, please call the office to request a replacement lab order.  It may take up to 3 days to process your lab replacement request so please plan accordingly.  We will call you when it is ready for you to pick up at the office.

Referrals
Please bring a referral or an authorization number from your primary care physician if your insurance requires one at the time of your appointment or your appointment will need to be rescheduled.

Primary Care Physician
The FDEC providers render consultative care and they will work in collaboration with your primary care physician. You will be required to have and maintain a primary care physician.  If you change your primary care physician, please inform the clinical staff at your next appointment.

Timeliness
It is crucial to the flow of the clinic that you arrive 20 minutes ahead of your scheduled appointment. Any arrival that occurs after your scheduled appointment time may be converted to a no-show and will be rescheduled. We also expect our front desk staff and providers to maintain urgency around minimizing your wait time. Our clinic goal is a door-to-door time of less than one hour. 

Preparedness

No-Shows / Late Cancellations / Rescheduling
If you are unable to make your scheduled appointment, it is your responsibility to notify us either by phone or by patient portal no later than 24 hours prior to your scheduled time, as this will allow us to offer your appointment to another patient. Any late notification will be converted to a no-show in the system. If, on the other hand, your appointment is cancelled due to a clinic or provider conflict, you will be offered several alternate clinic times within two weeks of your original appointment.

Prescriptions
Medications will be ordered either in the context of an appointment or by direct electronic request from your pharmacy. Refusal of refill requests may occur if you have not maintained an appropriate follow-up frequency.

Phone Calls

In order to better assist you, please utilize the patient portal and try limiting your calls to those involving non-urgent matters only. Any urgent issues should be directed to a local CentraCare or another urgent care center. As always, immediately call 9-1-1 with any and all emergencies.
